Doggie Daycare Interview Request Form
This form is a request to get your dog interviewed to join our Doggie Daycare program. Before you fill out this form, please be aware of our programs specific requirements:
- All dogs in the Doggie Daycare program are required to be up-to-date on their Bordetella, Rabies, Flu(H3N2/H3N8), and Distemper Combo vaccines.
- Dogs over 6 months old must be spayed/neutered
- Dogs in the Doggie Daycare program must come in at least once per week so the dogs in the program stay familiar with each other and we minimize the chance of injury/fights.
- Doggie Daycare hours are Monday - Friday, 8:00AM - 6:00PM
- If your dog passes the temperament test aka interview (which will usually take 10-20 minutes) they will then have to complete a trial day
- Their trial day is to be a 1/2 day (4 hours or less)
- Once they complete their trial day, your pet is ready to start in the program
